Types of updates
Update options may vary depending on how you update your personal information.
You may be able to make the following types of updates to your Health Card information online:
- change a name
- change a date of birth
- change an address
- change a phone number
- update a decision to donate organs and tissues after death
- report that you’re leaving Nova Scotia for a short time (temporary absence) or permanently
You need to contact MSI Resident Services to make the following types of updates to your Health Card information:
- add or remove a family member, child or dependant
- not display a sex indicator on your Health Card
- report a death
You can make the following types of updates to your Health Card information through the Health Card Change Form PDF:
- change a name
- change a date of birth
- change an address
- change a phone number
- change the sex indicator that displays on your Health Card
- update a decision to donate organs and tissues after death
Address consent and dependant primary contact
You need to use Address Consent to authorize your spouse or common-law partner to update your address (or use Revoke Address Consent to withdraw your consent). You need to use Dependant Primary Contact Change to change the primary contact for your dependants who are 18 or younger.
Who can use this form
You can use this form to update your personal information if your Health Card isn’t expired or doesn’t expire within 12 months.
If your Health Card expired more than 12 months ago or expires within 12 months, you need to contact MSI Resident Services to update your personal information.
What you need to do
- Complete the form online.
- Check the form for details on all required supporting documents.
- Submit your completed form and supporting documents.
How long it takes
It should take 4 to 6 weeks to update your Health Card information (and receive a new card if needed). Timing may vary depending on how you update your personal information and the type of update. It can take longer if more information is needed or if your form hasn’t been filled in correctly.
If you contact MSI Resident Services to change an address or phone number, your Health Card information is updated right away.
Cost
There is no cost to update your personal information.
Before you start
Make sure your card isn’t expired or due to expire within the next 12 months.
Online
When you submit the form online you need to create an account or sign in.
You can use the Health Card Change Form (PDF 253 kB) if you’re unable to complete the form online. Send your completed form and supporting documents by mail or fax. Or drop them off at MSI Resident Services (use the secure drop box; in-person service isn’t available).
You can contact MSI Resident Services if you’re unable to mail, fax or drop off the form.
If you use the PDF to update your personal information, make sure you have your Health Card number.
